Use this only when you are chasing someone or you plan to run away.
Right before going into duel.
Extra attacks always helpful in duels. You can go for attack speed skill first if you get into fights too much.
Use only when you are sure you will win. You don't want to give enemy the extra damage.

Use to get people unaware or at low HP with the ulti.

End Game (Winning)

You might also want to consider Heaven's Halberd in this one's place.
If you get stunned or slowed by abilites too much.
This or Halberd.
Use before combat to get some strikes in without getting hit, then use ulti. This or S&Y.

End Game (Losing)

Really worth it if an enemy hero is hitting too much but doesn't have as much HP as you do (such as phantom assassin)

Other Items

Either this and Daedalus or Mjollnir and Vladimir. Mjollnir and Daedalus is also a viable choice but keep enemy heroes in mind.
